Admissions

At UF Health Jacksonville, any physician can arrange for your treatment, and our staff will work closely with your doctor's office to facilitate your admission.

Our staff in the Admissions Office will call you prior to the day of admission to verify information. Your physician or the operating room staff will notify you of the time to arrive.

If you need financial assistance, you may be asked to provide employer information, proof of residency in Duval County, income statements and/or credit references. When you arrive, you will be asked to present your insurance cards and sign necessary paperwork.

The Admissions Office is open Monday through Friday, 5 a.m. to 5 p.m. and may be reached at (904) 244-7192.

What to bring:

  • Advance directives (living will, power of attorney, etc.)
  • Driver’s licenses and/or photo ID cards of the patient and the insured (if different from the patient)
  • Emergency contact information
  • Insurance card and/or policy
  • List of current medications — over-the-counter and prescription drugs, vitamins and herbal supplements
  • Medicare or Medicaid card
  • Physician’s orders, if you have any
  • Social Security numbers of the patient and the insured (if different from the patient)

To make your stay more comfortable, you are welcome to bring these personal items with you:

  • Bathrobe
  • Denture care supplies
  • Eyeglasses and case
  • Personal toiletries such as comb, brush, shampoo, soap, contact lens solution, toothbrush and toothpaste
  • Slippers

UF Health Jacksonville cannot be responsible for replacement of any personal belongings.

What to leave at home:

  • Valuables, such as jewelry, credit cards and cash (or give them to a relative or friend to take care of during your stay). If you cannot send your valuables home, speak with your nurse about storing them safely with our Security department.
  • Medications (prescription, over-the-counter or illegal drugs) from home. All medications you take while a patient at UF Health Jacksonville have been prescribed by your physician, are dispensed by the hospital pharmacy and are administered by a trained professional.
  • Electrical appliances, such as hair dryers, curling irons, razors, radios, heating pads, fans, portable heaters and other devices. These items are not permitted in patient rooms.

Planning for discharge:

Your physician will write a discharge order for you to return home or transfer to another facility when you no longer require hospitalization. After your nurse reviews your discharge instructions with you, you will receive a signed copy to take with you. You should make arrangements with a family member or friend to provide you with transportation upon discharge. Planning for discharge begins during admission, and patients and caregivers are encouraged to take part in the process.

Important considerations:

  • Will you be able to return to your pre-admission situation?
  • Has there been a change in your ability to care yourself?
  • When will you be discharged?
  • Will you be in need of services to be able to care for yourself after discharge?
  • Who will pick you up when you are discharged?
  • Do you and/or your caregiver understand the care, services or equipment you may need after you leave?
  • Do you have additional questions? Ask to speak to a case manager if you have need more help with your discharge planning.

Other steps before you leave:

  • Check your room, bathroom, closet and bedside table for any personal items.
  • Pick up any valuables you may have stored in the hospital safe.
  • Take your discharge folder with you for keeping all of your hospital paperwork, including discharge instructions, prescriptions and any referrals from your doctor.

Since discharge planning process can be overwhelming, patients have access to a case manager to assist in setting up services including alternative placement, home care, medical equipment, community services, counseling and transportation. Call (904) 244-4133 between the hours of 8:30 a.m. and 5 p.m. or ask your nurse to call a case manager for you.

A random selection of our patients will be contacted by phone to participate in a patient satisfaction survey. If you're feeling up to it, please take the time to give the surveyor your honest feedback.
