UF Health Home Care – Jacksonville provides compassionate,
in-home care services needed when someone is ill, but wants to remain at home.
UF Health Home Care – Jacksonville is a private, not-for-profit agency serving
Duval, Clay, Nassau, St. Johns and Baker counties. Our organization is Medicare-certified,
state licensed and accredited by The Joint Commission. UF Health Home Care is dedicated
to providing the highest standards of care. Our staff of health care professionals
offers home health services that promote independence and enhance an individual's
ability to remain in their own home and community with family and friends while
minimizing the loss of dignity that often accompanies illness.
Services offered include:
- Skilled nursing
- Physical therapy
- Occupational therapy
- Medical social work
- Home health aide
- Daily telehealth monitoring
- Speech therapy
While anyone may refer a patient for home health care, all care must be rendered
under the orders of a licensed physician. Typically, a nurse or other health care
professional visits the home, performs an evaluation, and under the direction and
review of the physician, prepares a plan of treatment.
UF Health Home Care – Jacksonville accepts assignment on Medicare and Medicaid
and most private group health insurance. Office hours are Monday through Friday,
8 a.m. to 5 p.m. Service personnel are available on-call seven days a week and an
answering service is provided 24 hours a day.
