The transition from pediatric to adult-oriented health care can be very complicated for people with disabilities and chronic health conditions. The Jacksonville Health and Transition Services, or JaxHATS, clinic was created to help make this transition easier. JaxHATS serves teens and young adults, ages 16-26, with chronic medical or developmental problems. Services are open to residents of Duval, Nassau, Baker, Clay and St. Johns counties in Florida.

To make a payment in person, visit our Cashier’s Office located on the first floor near the Emergency Department in the UF Health Jacksonville Clinical Center. Hours are Monday through Friday, 8 a.m. to 4 p.m.
