Your medical record is an important part of your health care experience. You have several options to making a request for your medical records. Patient Relations
Our Patient Relations advocates are available to assist you. Their office is open Monday through Friday, 7:30 a.m. to 5 p.m. They can be reached while you are in the hospital or after you have been discharged by calling (904) 427-4427 or (904) 244-4427.
