UF Health Jacksonville notifies patients of privacy breach

UF Health Jacksonville officials have notified 230 individuals affected by a privacy incident involving inadvertent address changes of the patient, guarantor and insurance coverage information caused by its electronic health record system. Incorrect address information caused pieces of correspondence to go out to the wrong individual.

This information did not involve Social Security Number, credit card or banking account information. Some demographic information, such as name, mailing address, phone number, date of birth, hospital account number, medical record number, account charges and insurance information, and clinical information from visits or appointments, such as physician name, date of service, type of visit and department location, may have been disclosed.

Upon learning of this incident on March 26, UF Health Jacksonville officials and the Privacy Office immediately began an investigation to determine the information disclosed. UF Health Jacksonville has no reason to believe that the information was used or misused; however, as a precautionary measure, affected individuals are being offered one year of complimentary credit monitoring. Letters were mailed this week to patients directly affected by this incident. UF Health Jacksonville also set up a call center to provide additional information or address any questions or concerns individuals may have. The call center is available Monday through Friday from 9 a.m. to 9 p.m. Eastern Standard Time at 1-833-903-3648, with voice response and live agents to assist.
