Medical Insurance
|
Full and part-time employees working 40 or more hours per pay period* (0.5 FTE or above).
Benefits begin the first day of employment.
PRN employees with at least one year of service and who have worked an average of 30 hours a week for 12 months.

Includes an EPO, a PPO, and a High Deductible PPO with HRA option under Florida Blue GatorCare.
Medical premiums are collected on a pre-tax basis and a Wellness Credit can be earned each year if you are enrolled in a UF Health medical plan.
Prescription coverage is included with all the medical plan options and is administered by Magellan Pharmacy Solutions.

Dental Insurance

Includes PPO and DMO plans with 100 percent preventative coverage after office visit co-pay. No claim forms required unless you choose an out-of-network provider.

Vision Care Plan
|
Full and part-time employees working 40 or more hours per pay period*.
Benefits begin the first day of employment.
|
Includes three options: Fashion Value, Designer Gold and Ideal Choice. Plans cover frames, lenses and scratch-resistant coating. Frames and lenses replaced once per year and discounts offered for Lasik surgery.
There are no calendar year maximums.

Basic Group Life
|
Full and part-time employees working 40 or more hours per pay period*.
Benefits begin the first day of employment.

UF Health Jacksonville provides employees with life insurance valued at one times their annual salary. Employees can also purchase an additional one, two, or three times annual salary.

Long Term Disability
|
Full and part-time employees working 40 or more hours per pay period* after one year of service.
Eligibility begins the first day of the month following an employee's one-year anniversary.

Paid to employee in the event of a disability, on or off the job, after 180 days.

401(k) Retirement Plan
|
Full time, part time and PRN employees.
|
UF Health Jacksonville offers both a traditional and Roth 401k. Employees are automatically enrolled at a 3% pre-tax salary deferral rate on their first day of employment and can elect to increase or decrease that rate at any time.
After 12 months of service (and at least 1000 hours), UF Health Jacksonville will match your salary deferral $1 for $1 up to 6%.

Allows employee to set aside pre-tax dollars to pay for eligible medical expenses up to $2,550 and dependent care expenses up to $5,000 per year.

Tuition Reimbursement

Full and part-time employees working 40 or more hours per pay period* and who have completed 6 months of employment.

Assistance available to employees enrolled in courses that will benefit them in their current or prospective position at UF Health Jacksonville.
Grade C or better = 100 percent reimbursement
Textbooks are reimbursed at 50 percent
Max benefit per fiscal year (July 1-June 30):
