Providers at UF Health Community and Family Medicine – Jacksonville offer comprehensive primary care services for patients of all ages, from pediatrics to geriatrics. Our dedicated team of medical doctors, nurse practitioners, pharmacists and a psychologist work together to provide whole mind-body care.
Services include preventive care, routine pap smear screenings, as well as management of chronic health conditions, such as diabetes, hyperlipidemia (high cholesterol), hypertension (high blood pressure) and obesity.
Clinical pharmacists are a part of the medical care team providing medication management for conditions, including diabetes and education management, high blood pressure, pulmonary and smoking cessation.
We also offer counseling services for ages 12 and older. Mental health conditions for counseling include anxiety, depression and sexual trauma.
Patient-Centered Medical Home
The National Committee for Quality Assurance has designated this practice a Patient-Centered Medical Home provider – a model for care where providers, patients, family members and caregivers partner together to achieve better outcomes.
