UF Health Total Care Clinic – Jacksonville

The UF Health Total Care Clinic – Jacksonville addresses the needs of patients whose care is rendered through the City Contract Program. Patients seen at the Total Care Clinic have a panel of doctors, nurses and other providers dedicated to maintaining and improving their health as well as assisting them with specialty care. On-site case managers will coordinate and track care of patients, especially those who have complex and/or chronic medical conditions.

• Psychiatry and mental health
• Pharmacist-assisted medication management
• Health education for chronic medical conditions such as diabetes, heart disease and malnutrition
