All of the following are required to be complete for your application to a PGY-1 or PGY-2 pharmacy residency at UF Health Jacksonville:

  • Applicant information
  • Letter of intent (address to program director, typically about one page in length)
  • Curriculum vitae
  • Three (3) letters of reference from professional sources
  • Official college transcripts (for pharmacy coursework)

Application materials for all programs should be submitted through PhORCAS.
