Ambulatory Care (PGY-2)
Since 1989, the PGY-2 Ambulatory Care Residency Program at UF Health Jacksonville has offered a diverse experience in patient focused care. Residents are integrated in our medical home, primary care and specialty clinics.
Knowledgeable preceptors and collegial relationships with the medical team are just a few of the opportunities. As the distance campus for the University of Florida College of Pharmacy, teaching opportunities and a teaching certificate program are also available.
Purpose and Goals
PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.
The goals of the UF Health Jacksonville PGY2 Ambulatory Care Residency Program are to develop in our residents the skills necessary to:
- effectively educate patients, students, and health care professionals
- prevent, identify, and resolve drug related problems and assure safe, effective, and appropriate drug therapy
- function as a member of a health care team as a confident, competent, and compassionate practitioner
- demonstrate leadership through a positive professional attitude and promote the profession of pharmacy
- self-assess professional growth and development
- conduct research to improve medication use and pharmacy practice
Program
The PGY-2 Ambulatory Care program provides the resident the education, training, and experience necessary to actively participate in a comprehensive ambulatory care setting. In collaboration with the medical staff, the resident provides care to a variety of patients on a daily basis.
Program Structure
The residency program provides training in all aspects of ambulatory care practice. To accomplish this, the resident will spend 4 weeks orienting (if new to the organization) or 2 weeks (if early commit) to the department and major hospital departments, complete core learning experiences and elective learning experiences; thus allowing the resident enough flexibility to pursue their individual goals. Completion of a research project and Grand Rounds presentation are also required. Learning experiences are 4 weeks in duration (1 block) with exception of the “holiday” block which is treated as one 8 week block (exceptions upon approval). Longitudinal learning experiences are 52 weeks in duration.

- Internal Medicine (Anticoagulation, Hypertension, Pain Management, Diabetes, MTM, Smoking Cessation) (2 blocks)
- Multispecialty IM (Nephrology/Transplant, Rheumatology, GI, Specialty Operations) (1 block)
- Neurology (1 block) (scheduled with Hepatology)
- Primary Care – CB McIntosh (Anticoagulation, Pharmacotherapy, Pain Management, Smoking Cessation, MTM) (2 blocks)
- Pulmonology (1 block) (scheduled with Cardiology)
- Total Care Clinic/Soutel Wellness (Anticoagulation, Hyperlipidemia, MTM) (2 blocks, combined with a primary care practice)
- Elective Learning Experience (1 block, 2 if early commit)

They also develop leadership skills through participation in professional organizations, mentoring PGY-1 residents and pharmacy students, and clinic development or expansion. Opportunities to educate patients, students, and other health-care professionals are integrated into each experience.
The resident also gains operational experience in a high volume, fully-automated outpatient pharmacy serving a diverse patient population and participates in our URAC and ACHC accredited specialty pharmacy operations. Areas of emphasis include formulary management, optimization of pharmacy operations, and managing drug shortages. The program is accredited by the American Society of Heath-System Pharmacists.

Application Requirements
Eligibility requirements for the residency program include the following:
- Graduate from an ACPE-accredited college or school of pharmacy with a Doctor of Pharmacy degree
- Completion of an ASHP-accredited Pharmacy (PGY-1) Residency
- Eligible for licensure in the State of Florida – all residents must take their exams prior to the start of the residency
- Interview
- For non-resident alien applicants, authorization for employment that is valid through the residency period is required

Kethia Robinson Dawkins, Pharm.D., BCACP is a clinical specialist in the Internal Medicine Clinic at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Robinson received her BS.Pharm. from the Massachusetts College of Pharmacy and her Pharm.D. from the University of Florida. She completed her Primary Care Pharmacy Practice Residency at UF Health Jacksonville.
Her areas of interest in practice include anticoagulation and medication error prevention.
Joseph Cammilleri, Pharm.D., BCACP, CPE
Joseph Cammilleri, Pharm.D., BCACP, CPE is a clinical specialist in Ambulatory Care Pain Management at UF Health Jacksonville and a Clinical Assistant Professor with the University of Florida College of Pharmacy.
Dr. Cammilleri received his Pharm.D. from the Palm Beach Atlantic University. He completed his PGY-1 Residency at UF Health Jacksonville in Jacksonville, Florida.
Dr. Cammilleri is the chair of the UF Health Jacksonville Department of Pharmacy Research Committee and an active member of UF Health Jacksonville Pain Management Team Committee. He is also a member of the Northeast Florida Society of Health-System Pharmacists.
His areas of interest in practice include pain management, sickle cell anemia, smoking cessation, and iron chelation.
Lanh Dang, Pharm.D., BCACP
Lanh Dang, Pharm.D., BCACP is a clinical specialist in Ambulatory Care Multispecialty at UF Health Jacksonville and a Clinical Assistant Professor with the University of Florida College of Pharmacy.
Dr. Dang received her Pharm.D. from the University of Rhode Island. She completed her PGY1 Residency at UF Health Jacksonville and stayed on for her PGY2 in Ambulatory Care. Dr. Dang is an active member of Northeast Florida Society of Health-System Pharmacists, Florida Society of Health-System Pharmacists, and American College of Clinical Pharmacy. Within the Department of Pharmacy, she is actively working alongside her colleagues to obtain specialty accreditation.
Her areas of interest include specialty medication processes, inflammatory bowel disease, and primary care.
Denise Darracott, Pharm.D., BCACP, CDE
Denise Darracott, Pharm.D., BCACP, CDE is a clinical specialist in Ambulatory Care at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Darracott received her Pharm.D. from the University of Georgia, College of Pharmacy. She completed her pharmacy practice residency at Shands Jacksonville Medical Center. She is a member of the American Society of Health System Pharmacists, the Florida Society of Health-System Pharmacists and the local Northeast Florida chapter.
Her areas of interest in practice include anticoagulation, diabetes management and medication therapy management.
Julie Ferm, Pharm.D., BCPS
Julie Offutt Ferm, Pharm.D., BCPS is a clinical specialist in Outpatient Nephrology at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Ferm received her Pharm.D. from Butler University in Indianapolis, Indiana. She completed her pharmacy practice residency at Clarian Health Partners in Indianapolis, Indiana and then completed her PGY2 Primary Care Residency at the University of Iowa Hospitals and Clinics in Iowa City, Iowa. She is active in Northeast Florida Society of Health System Pharmacists.
Her areas of interest in practice include nephrology, hemodialysis/peritoneal dialysis, and post kidney transplantation care.
Catrina Graham, Pharm.D., BCACP
Catrina Graham, Pharm.D., BCACP is a clinical specialist in the Neuroscience Institute at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy and Clinical Assistant Professor of Florida Agricultural and Mechanical University College of Pharmacy and Pharmaceutical Sciences.
Dr. Graham received her Pharm.D. from the Florida Agricultural and Mechanical University College of Pharmacy and Pharmaceutical Sciences. She completed her pharmacy residency at UF Health Jacksonville.
She is a member of the Northeast Florida Society of Health-Systems Pharmacists.
Her areas of interest in practice include strokes/cerebrovascular disease, multiple sclerosis, peripheral neuropathy, seizures/epilepsy, headaches (migraine, chronic daily), Parkinson’s disease, neuromuscular disorders and tremors.
Taylor Herndon, Pharm.D
Taylor Herndon, Pharm.D. is a clinical specialist in Ambulatory Care Gastroenterology and Dermatology and a Clinical Assistant Professor with the University of Florida College of Pharmacy.
Dr. Herndon received her Pharm.D. from the University of Georgia College of Pharmacy. She completed both her PGY1 Residency and PGY2 Ambulatory Care Residency at UF Health Jacksonville. Dr. Herndon is a member of the Northeast Florida Society of Health-System Pharmacists, Florida Society of Health-System Pharmacists, and Georgia Society of Health-System Pharmacists.
Her areas of interest include dermatology, gastroenterology, dyslipidemia, and specialty medication processes.
Katherine Hernandez, Pharm.D.
Katherine Hernandez, Pharm.D. is a clinical specialist in Ambulatory Care at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Hernandez received her Pharm.D. from the University of Florida College of Pharmacy. She completed her PGY1 pharmacy residency focused in ambulatory care at the University of Florida College of Pharmacy. She is a member of the Florida Society of Health-System Pharmacists and the local Northeast Florida chapter.
Her areas of interest in practice includes diabetes, transitions of care, and anticoagulation.
Abbey Johnston, Pharm.D., BCACP
Abbey Johnston, Pharm.D., BCACP is a clinical specialist in Ambulatory Care - Gastroenterology and Hepatology Specialty at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Johnston received her Pharm.D. from the University of Florida College of Pharmacy Jacksonville campus. She completed a PGY1 Pharmacy Practice residency and PGY2 Ambulatory Care Practice residency at UF Health Jacksonville. Dr. Johnston is a member of the American Society of Health-System Pharmacists and Northeast Florida Society of Health-System Pharmacists.
Her areas of interest in practice include hepatitis C, inflammatory bowel disease, specialty practice and ambulatory care practice and systems.
Billie Nguyen, Pharm.D.
Billie Nhu Nguyen, Pharm.D., serves as Manager of Ambulatory Care Pharmacy Services at UF Health Jacksonville. In this role, she oversees the Ambulatory Pharmacy, a 340B pharmacy that fills approximately 400,000 prescriptions annually, clinical pharmacy services in numerous primary care and specialty outpatient clinics, and the specialty pharmacy services. She is also a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Nguyen received her Pharm.D. from the University of Florida College of Pharmacy. She completed her two-year pharmacy fellowship in Family Medicine at the UF Family Practice Group.
She is an active member of ASHP, FSHP, and NEFSHP. Her areas of interest include Medication Therapy Management for chronic disease states (diabetes, anticoagulation, pain), pulmonary arterial hypertension, error prevention, medication reconciliation, transition of care, as well as using pharmacy and electronic health systems to enhance the practice of pharmacy.
Ashley Sargent, Pharm.D.
Ashley Sargent, Pharm.D. is a clinical specialist in outpatient Neurology at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Sargent received her Pharm.D from Thomas Jefferson University in Philadelphia, Pennsylvania. She completed her PGY1 Residency at Anne Arundel Medical Center in Annapolis, Maryland and then completed her PGY2 Ambulatory Care Residency at UF Health Jacksonville. She is an active member of the Florida Society of Health-System Pharmacists.
Her areas of interest include multiple sclerosis, demyelinating diseases, migraines, epilepsy/seizures, and cerebrovascular disease.
Brendia Toliver, Pharm.D.
Brendia Toliver, Pharm.D. is a clinical specialist in Primary Care at UF Health Jacksonville and a Clinical Assistant Professor of the University of Florida College of Pharmacy.
Dr. Toliver received her BS.Pharm. from Auburn University and her Pharm.D. from Florida A & M University. She completed her Primary Care Pharmacy Practice Residency in 2002 at UF Health Jacksonville.
She serves as Clinical Pharmacist Advisor for the Jacksonville Urban Disparities Institute (JUDI) /UF Health Jacksonville Community Clinics, where she collaborates with the medical director and chronic disease departmental chairs to provide oversight of nurse driven protocols and clinic guidelines for appropriateness of medication therapy. Dr. Toliver is active in the Florida Society of Health-System Pharmacists, Auburn University Pharmacy Alumni Association, and Florida A&M University Alumni Association.
Her areas of interest in practice include providing optimal pharmaceutical care in the areas of Anticoagulation, Hyperlipidemia, Hypertension, and Diabetes Management as Lead Clinical Pharmacist in the UF Commonwealth Group NCQA recognized Patient Centered Medical Home practice. Dr. Toliver enjoys participating in the Alliance for Integrated Medication Management’s National Patient Safety and Clinical Pharmacy Services Collaborative in an effort to improve the delivery and documentation of clinical pharmacy services and increase patient safety by an inter-professional team approach. Dr. Toliver is also actively involved in health disparities research.
Justin Tinsley, Pharm.D., BCACP
Justin Tinsley, Pharm.D., BCACP is a clinical specialist in Ambulatory Care at UF Health Jacksonville and the PGY2 Ambulatory Care Residency Program Director. He serves as a Clinical Assistant Professor of the University of Florida College of Pharmacy as well as a Clinical Instructor of Pharmacy Practice for Palm Beach Atlantic University.
Dr. Tinsley received his Pharm.D. from the Lloyd L. Gregory School of Pharmacy at Palm Beach Atlantic University and later completed his PGY-1 pharmacy practice residency with an emphasis in primary care at the Ralph H. Johnson Veterans Affairs Medical Center in Charleston, South Carolina.
He has an active membership in the Northeast Florida chapter of Health-System Pharmacists and is a founding father of the Delta Upsilon chapter of Kappa Psi at Palm Beach Atlantic University.
His areas of interest in practice include anticoagulation and cardiology.
Shannon Usztok, Pharm.D., BCACP
Shannon Usztok, Pharm.D., BCACP is a clinical specialist in Ambulatory Care at the UF Health Jacksonville C.B. McIntosh Center and a Clinical Assistant Professor with the University of Florida College of Pharmacy.
Dr. Usztok received her Pharm.D. from the University of South Florida College of Pharmacy. She completed her PGY-1 Pharmacy Practice residency and PGY-2 Ambulatory Care residency at the James A. Haley Veterans’ Hospital in Tampa, FL. Dr. Usztok is an active member of Kappa Psi Pharmaceutical Fraternity, Inc. where she serves as the Vice Regent for the Jacksonville Graduate Chapter, Co-Grand Council Deputy of the Zeta Gamma Chapter in Jacksonville, and Assistant Supervisor of the Gulf Coast Province. Dr. Usztok is also a member of the Northeast Florida Society of Health-System Pharmacists.
Her areas of interests in practice include diabetes, hypertension, dyslipidemia, anticoagulation and smoking cessation.
